This website is operated by Meridian Care Solutions LLC dba Illinois Personal Injury Centers, in the State of Illinois ("we", "us"). By using this site, contacting us or sending us a form, you agree to these terms.

What we do

We are an injury case management and scheduling service. Our coordinators schedule injured people with independently licensed providers, help keep appointments and files organized, and can put a person in touch with an independent attorney on request.

Not medical advice

Information on this site is general and is not medical advice. We are not a medical provider. Care is delivered by independently licensed providers, who alone decide what care is appropriate.

Not a law firm and not legal advice

We are not a law firm. Nothing on this site or said by our staff is legal advice. Legal services, where referenced, are provided only by independent attorneys. Contacting us does not create an attorney-client relationship. That relationship exists only if you sign a written agreement directly with an attorney.

Not an emergency service

This site and our phone line are not emergency services. If you have a medical emergency, call 911 or go to the nearest emergency room.

Independent providers and attorneys

The providers we schedule you with, and any attorney we put you in touch with, are independent professionals. They are not our employees or agents. Your relationship with any provider or attorney is between you and that professional, and we are not responsible for the care or representation they deliver or for its outcome.

No guarantees

We do not guarantee that a provider or attorney will be available, that one will accept you as a patient or client, or any particular medical, legal or financial result.

Who can use this site

You must be 18 or older to use this site or send us a form. A parent or guardian can contact us on a minor's behalf.

Accurate information

You agree to give us accurate information and to tell us if it changes, and not to misuse the site or interfere with its operation.

Text messages and calls

If you give us your mobile number and agree to receive text messages, we may text you about your request and your appointments. Message frequency varies. Message and data rates may apply. Reply STOP to stop texts or HELP for help. Agreeing to receive texts is not a condition of being scheduled. Carriers are not liable for delayed or undelivered messages.

Content and ownership

The text, design and logos on this site belong to us and may not be copied or reused without our written permission. Site content is provided "as is" for general information. We work to keep it current but do not promise it is complete or error-free.

Limitation of liability

To the extent permitted by law, we are not liable for indirect, incidental or consequential damages arising from use of this site or our scheduling service.

Governing law

These terms are governed by the laws of the State of Illinois. Any dispute will be brought in the state or federal courts located in Cook County, Illinois. Please contact us first so we can try to resolve it.

General

If any part of these terms cannot be enforced, the rest stays in effect. These terms and our Privacy Policy are the full agreement between you and us about using this site.

Changes

We may update these terms from time to time. The current version is always posted on this page with its effective date.
